#WeekendCoffeeShare (November 30, 2024)

Hi everyone on this last day of November. I’m joining #WeekendCoffeeShare. I just had my last cup of coffee for the day, with a treat of the Dutch typical St. Nicholas candy mix “strooigoed”. I’d have loved to pass you some, but ate the whole bag myself. I’m just comforting myself that the amount wasn’t nearly what I used to consume during a binge. Anyway, let’s have a drink and let’s catch up.

If we were having coffee, first I’d talk about the weather. A serious storm passed by here on Wednesday. It was the first major storm in nearly a year and a half or so I read on the news. That surprised me, since it wasn’t even a year ago that a staff from one of the intensive support homes died while riding the side-by-side bike with a client. Then again, the most severe winds aren’t in this province and therefore maybe near the coast people are less likely to go out in stormy weather.

Anyway, during the remainder of the week, it hasn’t rained and sometimes even the sun came out for a bit. Daytime temperatures have been around 8°C.

If we were having coffee, I’d say that I’m doing pretty well this week. I consider the fact that this is my fourth blog post of the week, a positive sign of my improved mental health. I really hope it continues.

If we were having coffee, then I’d share that I have a nasty cold. This morning, I spent most of the time in bed because I was feeling so miserable. No fever, thankfully, but in my case even a cold can knock me down for a few days.

If we were having coffee, then I’d tell you that I’ve been quite crafty over the week. On Tuesday, I created a polymer clay pig for a staff who will celebrate 25 years of working for this care agency on Monday. And just so you know, she was the one constantly asking me if I could create a pig; it isn’t like I think a pig is especially suited to her.

On Thursday, I crafted yet another polymer clay unicorn, this one for a new staff, who is into unicorns as much as I am. I tried adding flower-shaped wings to it, but this wasn’t a success, since during the curing process, one of the wings fell slightly down the back of the unicorn and got distorted in the process. No photo, sorry, since I threw the thing into the bin.

Then today, I didn’t know what to do, so I decided to mix a polymer clay color. Not sure what I’ll do with it.

If we were having coffee, I’d share that I didn’t do Black Friday shopping this year. That is, I did purchase some new clay supplies yesterday, but they weren’t on sale as far as I could tell.

If we were having coffee, lastly I’d tell you that we had pasta salad with tuna for dinner today. I had originally planned to make it with a staff, but since I don’t want everyone else to catch my cold, the staff ended up making it himself. He initially wanted to offer everyone a bit as a side dish with our regular meal, but I countered that we have enough to use it as a meal. My plan had been that we’d have a tasty meal for once rather than the mediocre meal service food. Besides, we get treats more often than I want to think of. See my point about the St. Nicholas treat: it wasn’t a binge, but it was more than my food plan says I can have and I’m not even on a weight loss diet.

How I Coped With Losing My “Job”

Hi everyone. This week, one of the prompts for Writer’s Workshop is to write an essay titled “How I coped with losing my job”. I don’t do well writing fictional essays and have never had a “real” job, in the sense of a paid position or even volunteer work. I did, however, once “lose my job”, in that I got told the day center I went to couldn’t keep me there anymore.

This happened sometime in January of 2018 at my first day center with my current care agency. They had had me there for eight months, in two different groups, but when I was struggling to cope at the second group, they could no longer serve me or so they said.

I felt really distressed about this. The most frustrating aspect was the fact that they blamed me for no longer being suited to the center, while in reality, three new clients had been accepted into my group and no additional staff had been hired. I remember the reason they said it was me being the problem, not the new clients, was the fact that I’d been having meltdowns shortly before they arrived. Now I know that any anticipated change will cause me distress and that doesn’t mean I’m just a problem client. However, in hindsight, I’m pretty sure they wanted me gone sooner rather than later all along, for the simple reason that I don’t have an intellectual disability.

Thankfully, I wasn’t told to leave on the spot, but got time to find a new place. I initially had no clue how to, but did remember that, in 2010, I had been helped by the Center for Consultation and Expertise (CCE). I told the staff that I wanted to involve them again.

This was a bit of a hassle, as my community psychiatric nurse from the mental health agency said I’m far too high-functioning for the CCE. I applied nonetheless and got an orientation meeting in May of 2018.

In the end, I didn’t need the CCE for finding a new day center, but the consultation was what led me to accept that living independently with my partner wasn’t working and I needed long-term care.

I did feel intensely frustrated, like I said, at being “fired” from this day center. However, in the end, I don’t blame the staff, who were just powerless in the face of my challenging behavior. I think the manager, who didn’t look beyond my psychiatric diagnoses, is partly responsible. So was the psychologist from the psychiatric hospital, who more or less made the manager accept me on partly false premises.

#WeekendCoffeeShare (May 11, 2024)

Hi everyone. Today I’m joining #WeekendCoffeeShare. It’s been a long week, honestly, so time for a cup of coffee, green tea or a smoothie. Let’s have a drink and let’s catch up.

If we were having coffee, first I’d talk about the weather. It’s been quite warm and sunny most days, with daytime highs around 20°C. Tomorrow, the temperature is even supposed to reach 25°C. The mornings have been cooler though, as it’s not yet July, obviously. Which makes me think, with temperatures like this in May, will we get a soaring hot summer again? Most likely. I love warm weather, but it’s not like I want summertime temps over 35°C.

If we were having coffee, I’d tell you I’ve been walking a fair bit over the past week. I also rode the side-by-side bike yesterday. We finally figured out how to make it so that I can actually push the pedals properly rather than just moving along with the person who’s on the steering side of the bike.

If we were having coffee, I would tell you that, on today’s morning walk, we came by the intensive support home and several clients were calling out to me. I decided to come over and join them in their backyard. Eventually, one of the staff who did work there when I still lived there, joined us and offered me a cup of coffee. I think that’s nice!

If we were having coffee, I’d share that, finally, it looks like my orthopedic shoes are actually good to wear. The last adjustment was to the front, which scratches the ground due to my ever-worsening drop foot, causing the shoe to need repairing almost on a weekly basis. Now, it’s not like it doesn’t still scratch the ground and get damaged, but not nearly as much as it used to. And the big positive: I can actually wear these shoes without getting blisters. Like, okay, that’s normal, but I only used to be able to wear my specific brand of walking shoes and I’d get blisters from everything else, including every pair of orthopedic shoes I’ve tried before. I have actually been wearing these shoes most of the day for the past few days without a problem.

If we were having coffee, I’d report that, speaking of my drop foot, the physical therapist has been here. We went for a 20-minute walk, during which I didn’t walk with a drop foot as much as before, thankfully. It hasn’t had me convinced that my mobility isn’t worsening, but at least it isn’t as bad as I’d feared. I mean, it could just be normal aging with mild cerebral palsy.

If we were having coffee, I would share that I bought yet another pair of new headphones. Originally, I wasn’t intending on giving up on my AirPods, but because they keep losing connectivity to my computer, I needed headphones that come with a wired option. Well, guess what? The wired option for the JBL Tune 770NC headphones isn’t all that awesome, but the headphones don’t lose connection to my computer when connected via Bluetooth. Besides, it can be connected to both my iPhone and computer at the same time. How I wish I’d known about these before buying the AirPods, that were literally three times the price of these headphones.

If we were having coffee, I’d share that I also finally replaced my desk chair. I got a gaming chair. That’s the reason I have been wearing my orthopedic shoes all day: the chair is too high for me to sit on without shoes on. Other than that though, it’s much better than my previous chair.

I also ordered a table and two chairs for in my little backyard. These weren’t in stock at the store I went to, so I’ll have to come back to collect them. While at that store, I saw a really cute unicorn soft toy and just had to get it.

If we were having coffee, lastly I’d admit that I have been struggling with my mental health again and, as a result, haven’t been too inspired in the creative department recently. I do really hope to get back into the groove soon. I did, last week, buy some kitchen tools, like measuring spoons and a sugar/flour sieve. I’ve only used the measuring spoons for smoothie making so far, but that’s a start at least.

My Hopes for 2024

Hi everyone. As regular readers of my blog know, I don’t do New Year’s resolutions, instead calling them “hopes”. This is just an excuse not to have to take responsibility, I guess, but oh well. I love writing them and looking back at them after a year to see how well I did. Last year, I did participate in #WBOYC, of course, but honestly I never looked at my hopes for 2023 when doing my monthly reflections. Oh well, maybe my first not-quite-resolution has to be to look back at this post when reflecting on the month. Here goes.

1. Get back on track with respect to healthy’ish eating. I did manage to get to a healthy BMI in 2023. That is, I’m pretty sure with all the New Year’s treats, including more “oliebollen” than I told my dietitian I liked, I’m now back across the line to overweight (I was only half a pound below that line last week). I know that with some cutting back on snacking, and it doesn’t even have to be drastic, I can lose the excessive pounds I gained over the holidays. More importantly though, for the remainder of the year, I’d like to maintain a relatively healthy diet. After all, sugary ttreats and high-fat foods are bad for you even if you’re at a healthy BMI.

2. Get back into a regular movement routine. Though I did excellent for the first half of 2023 and pretty well for most of the second half too, like I said yesterday, I wasn’t moving as much over the past month or two as I would’ve wanted to. This year, I obviously hope to meet my movement goal each day, but I’m not sure that will be a realistic goal, as it wasn’t in 2023 and that’s not just because of lack of motivation. I’d also really like to find another way of exercising besides walking, either swimming again or something else. I seem to remember we do have gym equipment somewhere on institution grounds, so maybe that’s an option.

3. Write more regularly. Last year really sucked in the blogging department. I really hope to change that and get back on track with blogging more often.

4. Broaden my horizons in the crafty department. I hardly touched on this yesterday, but the last couple of months were really positive in this respect. For one thing, I started creating figures, such as the gnome and Santa, without consulting YouTube tutorials. For another, on Saturday, I did something really cool: I crafted a unicorn completely independently! I did initially leave black streaks on the head from touching it while trying to attach the eyes, which the staff had to clean up with alcohol. Other than that, I did everything completely without supervision or help.

In 2024, I really hope to be more creative, try out more things on my own (without the expectation that I’ll lose my one-on-one) and generally have more fun claying and maybe doing other crafty things.

5. Explore other day activities. In 2023, I wanted to expand my horizons with respect to my day schedule, moving beyond 60 minutes at a time, beyond my room and beyond one-on-one. The first was a massive success, as I now have a great day schedule (which of course took me moving out of the home I lived in last year, but oh well). The second and third, I’m not 100% sure I want to achieve, actually. Rather, I’d like to explore meaningful activities, whether these can be done in my room or elsewhere, on my own, with my one-on-one or in a group.

6. Get settled into my current care home, generally. I really hope to get more comfortable here and start building up a trust-based relationship with some staff.

7. Improve in the mental health department. This, for me, means stabilizing with respect to my trauma-related symptoms. It also means getting closer to a proper med combo and dosage. In 2023, I didn’t change any of my meds or dosages, which was frustrating as I did notice some increase in possible side effects. I’ll have a meeting with the intellectual disability physician to discuss my meds on the 15th.

8. Continue to work on my relationships and supporting the people I love. Particularly, my marriage evolved in a positive way in 2023 and I would really love to keep it as strong as it is going forward.

What are your hopes for this year?

Before and After

I rarely if ever turn the pages of an actual book these days, since I can’t read print and Braille books are just too clunky to have around. Turning pages, for this reason, is mostly just a figure of speech: I can turn the page on a memory, turn pages in the book that is my life, etc.

Sixteen years ago today, I experienced a turning point in my life, as on that day, my fragile mental state completely collapsed. The night after, at roughly 2AM on November 3, 2007, I was admitted to the psychiatric hospital.

Since then, my life consists of a “before”, in which I appeared to more or less function in life according to non-disabled standards (but was really merely surviving), and an “after”, in which I appear to have given in to the disabled side of me (but am slowly learning to live). I struggle to unite the two.

The Wednesday HodgePodge (November 1, 2023)

Hi everyone. It’s Wednesday once again, so I’m joining in with the Wednesday HodgePodge. Here we go.

1. Besides Thanksgiving (in the USA) what’s one thing you’re looking forward to in November?
Not sure really. November is the hardest month of the year for me. My spouse’s birthday is this month, but I won’t be going to our house in Lobith for the weekend. I’m pretty sure we’ll find a way to celebrate though and that’s what I’ll be looking forward to.

2. Do you like candles? Your favorite scent? How often do you burn a candle in your home?
No, I don’t. They’re not safe for me because of the flame. I used to love wax melts though. My favorite scents were sweet scents reminiscent of bakeries like those including vanilla, cinnamon and coconut.

3. What gadgets did you use today?
My laptop, iPhone and Apple Watch.

4. This question is a repeat from one asked in November of 2014, but I liked it so it’s coming round again. Many of you weren’t here in 2014. Okay, you can have fifty pounds of something (anything but money)…what will you choose? Also, since I mentioned it…what were you up to in November of 2014?
Fifty pounds of polymer clay LOL. Then I could make some giant unicorns. Seriously though, I have absolutely no idea what substance it would be useful to have fifty pounds of. Except maybe gold so that I could trade it in for money, but that’d be cheating.

As for where I was in November of 2014, I was in the psychiatric hospital in Wolfheze. If I remember correctly, the psychologist who ended up kicking me out of there in 2017 had just become my responsible clinician.

5. ‘Tis the season…what’s something you’re feeling especially grateful for today?
My mental health. It’s November and I’m struggling, but not nearly as badly as I was last year.

6. Insert your own random thought here.
As I shared yesterday that I hoped I wouldn’t have gained significantly at my weigh-in today, I owe you all the result: I lost 0.5kg.

October 2023 Reflections #WBOYC

Hi everyone. It’s the last day of the month and this means I’m reflecting on the past month’s happenings. As usual, I’m joining in with What’s Been On Your Calendar? (#WBOYC).

First, I’m finally going to share the polymer clay unicorn I crafted on September 30. Okay, that’s not technically the past month, but I didn’t fully finish it and take a picture till October 5.

My spouse joked that, judging by its colors – fuchsia, yellow and blue (the blue is called Peppermint, don’t ask me why) -, it’s typical of a specific music scene from the nineties. The staff who helped me craft this unicorn, is only slightly older than me, so she understood.

I haven’t really been crafting with clay much over the month of October. I did though help cook dinner twice. I also made a few smoothies.

I also did a good amount of walking, although I didn’t meet my movement goal every single day this month. I blame the rain, because the one day I didn’t meet the goal, it was raining almost constantly. Besides walking, I went swimming once.

My mother-in-law visited me three times this month and my spouse came by each week. My sister had originally wanted to come by this Sunday, but I prefer not to see her or my parents in the institution. Instead, my spouse and I are going to see them and my parents at Christmas.

Mental health-wise, the month has been quite good, truthfully. I mean, I’m still adjusting to my new care home and it’s October, which is a hard month for me each year. Taking this into consideration, however, I can’t complain. I am intensely grateful for the fact that most staff go out of their way to accommodate me. Initially, I was told by some that I’d be assigned the temp worker almost by default, which set me off because that was exactly what happened at my old care home and, given my attachment issues, I struggle with this. I spiraled into a bad crisis for this reason last week. Thankfully though, the staff now try their best to assign at least a somewhat familiar staff to me if they have to be a temp worker after all.

With respect to my physical health, I am happy to report I didn’t lose any more weight. In fact, I gained a few pounds. It wasn’t like I definitely couldn’t lose any more weight for my health, but I would’ve felt concerned had I lost more weight, given how much I ate over the past month. I am now within the weight range I agreed upon with my dietitian rather than slightly below it, so I’ve decided I can no longer afford as many treats as I used to consume. Yesterday, I convinced my assigned staff to add my food plan to the manila folder of important information that’s on my table in my room. After all, staff would often hand me a cookie (or two) without even thinking about it with each coffee break, despite the fact that my food plan has one only with my evening coffee break. I am due for weigh-in tomorrow morning again. Fingers crossed I won’t have gained significantly.

Mental Health in People With Intellectual or Developmental Disabilities #AtoZChallenge

Hi everyone. I once again didn’t have time for writing my letter M post in the #AttoZChallenge yesterday, because I was at the countrywide cerebral palsy day and then at my in-laws and was too tired once I came back to the institution to write my post. Let me for this reason write it today. For my letter M post, I am going to write about mental health as it relates to people with intellectual and developmental disabilities.

People with an intellectual disability are more likely to have mental health problems, including severe mental illness, than the general population. However, in the general psychiatric system, these people are not usually adequately helped. This means that early recognition of people with an intellectual disability is very important. Here in the Netherlands, some psychiatrists actually advocate for administering a simple screening tool for mild intellectual disability to each person coming into care with significant mental health issues. That way, if a person is identified as potentially having an intellectual disability, treatment can be adapted for them.

Other issues in mental health services for people with intellectual disabilities include the need for more trauma-informed care, since intellectually disabled people are at increased risk of being victims of abuse. Of course, trauma treatment, as well as therapy in general, needs to be specifically adapted to meet the intellectually disabled person’s needs. With EMDR, this is possible even with severely intellectually disabled people. Other forms of treatment, such as dialectical behavior therapy and schema-focused therapy, are, with some modifications, useful for people with mild intellectual disability.

In most countries, people with mild intellectual disability are usually seen by general psychiatric providers. However, here in the Netherlands, at least some mental health agencies have specialized teams or even an entire separate agency serving those with mild intellectual disability and co-occurring mental health issues.

Though autism as a co-occurring developmental disability with mental illness really poses some of the same challenges as does intellectual disability, this is not widely recognized. I mean, most mental health agencies here do have autism teams, but these are often dedicated to diagnosis and short-term psychoeducational support of autistic adults. There are a few specialist treatment centers for autistics with highly complex needs due to comorbid mental illness and/or severe autism, but these are inpatient units with long waiting lists. As far as I’m aware, there hardly seems to be any outreach-based, long-term treatment specifically for autistics with complex care needs.

My Hopes for 2023

Hi everyone and a happy 2023 to you all! Regular readers of my blog know that I don’t do resolutions or new year’s goals. Instead, I call them hopes. Whether that isn’t just the exact same but worded slightly euphemistically, is up for debate. However, I found that, many years ago, when I did lists of resolutions, they didn’t work out. Now that I call them “hopes”, I usually probably subconsciously stick to them more. I say this every year as an introduction to my hopes.

This year, I haven’t actually thought of many hopes I have. That is, I do hope to achieve many things, but secretly I do think my “hopes” do need to be somewhat realistic in order to make it onto my new year’s list. Doesn’t that make them just like goals? Oh well, I guess so. With no further ado, I’m just going to write down the things I hope to achieve in 2023.

1. Get to a healthy weight. I never dreamt of writing this when I started my healthier eating journey at the beginning of 2022, but I lost over 10kg in 2022 and I’m only 3kg above a healthy BMI now.

2. Keep up my movement routine. I have my movement goal on my Apple Watch set pretty low at 300 active calories a day. I’d really like to surpass it regularly.

I would also like to improve on the parameters of my physical fitness level, such as my heart rate variability. These are all really low right now.

I am hoping to start actually exercising in other ways besides walking. I’d love to start swimming here on institution grounds.

3. Get to a more stable place mentally. In 2022, I hoped to remain stable mental health-wise, which got ruined by my choice to move and its consequences. Now, I’m hoping to get back to a place of relative calm again.

4. Further lower my antipsychotic dosage. I really hope the move to my current care home doesn’t mean I’m on 25mg of aripiprazole forever.

5. Get to a meaningful day structure and day activities. Okay, I have the new day schedule, which is better than the one my support coordinator gave me, but it’s not what I’d hoped for when I came here. I am really hoping to explore day activities beyond my room, beyond 60 minutes at a time and beyond one-on-one.

6. Write regularly. This is another thing I was pretty awesome at in 2021, hoped to maintain in 2022 and lost track of due to the move. I even only wrote five blog posts during the month of November. I am really hoping to get back into the writing groove this year.

7. Further explore my creative side. I’d really like to find ways to do part of my polymer clay work independently, so that I can actually do more complicated things with my 60-minute time slots of supported activity.

8. Socialize more. This was one of the main reasons for the move, at least according to the behavior specialists involved. Indeed, it is a positive aspect of my current home that wasn’t there at my old home. I honestly don’t know whether I can socialize much with the people at my home, but I can at least try and, if that doesn’t work out, there are almost 400 other residents here at the main institution. I’d really love to talk to some of them more.

I’d also love to connect to my peers in the cerebral palsy community more. I’ll hopefully attend the countrywide cerebral palsy day again in April and also hopefully join the online meetings more often. I’m also hoping to get in touch with the Eye Association more. I know this was something I hoped to achieve in 2022 but didn’t.

9. Get serious about the basics of my faith. I had a discussion about faith with my husband yesterday and the bottom line was that I tried to run without having learned to crawl, so to speak, because I was watching John MacArthur videos, which are deeply theological, even though I’m really still a new believer. I mean, okay, I’ve been a Jesus follower for two years, but I’m still struggling with the basic concepts of the Christian faith. That’s probably why I call myself a “progressive Jesus follower” rather than unapologetically claiming my identity as a Christian. I really hope to move closer on my journey towards God in 2023.

However, it’s past 9PM so I don’t think I’ll have the time or energy for another blog post.

Saying Goodbye to Mental Health Services

Hi everyone. October 10 is World Mental Health Day. In honor of this, many bloggers are writing about mental health this week. As a person with serious mental health challenges, I think I need to offer up my contribution too. However, mine will be a bit different, in that just last Tuesday I had my for now final appointment with a professional from a mental health service.

I did not usually share details of my less than stellar experiences with my psychiatric nurse practitioner on the blog, but let me say that my appointments, except for those concerning medication, over the past three years, have for the most part been useless. Maybe my expectations of an assertive community treatment team were too high. For those who don’t know, an assertive community treatment team is an outreach-based team that treats the most severely mentally ill, usually care-avoidant patients. As a result, a lot of the care they do is more like support and steering towards services than actual therapy. They do have a psychologist on the team, but the majority of clinicians are actually community psychiatric nurses.

Back when I got kicked out of the psych hospital in 2017, I did need such a team, as I needed lots of support in my daily life. However, once I’d moved into long-term care, maybe an assertive community treatment team wasn’t the most logical choice of treatment teams. And yet that’s where I ended up.

Not to say it’s all my or my referring agency’s fault. My nurse practitioner did flat out refuse to refer me anywhere else even when he should have realized he couldn’t provide the kind of treatment I needed. He also claimed we were doing “trauma treatment” by merely talking over my mental health struggles, which happen to be trauma-related, occasionally. In my opinion, it was neither specifically trauma-focused nor treatment.

Now I know that my staff and the behavior specialist at the care home are probably as capable as my mental health professionals were at helping me deal with my everyday instability. Neither can provide trauma-focused therapy, while both can provide a listening ear (my staff at my old care home at least often better than my nurse practitioner, in fact).

I did, of course, start my topiramate and pregabalin while in treatment with my now former mental health team. This I do credit the team’s psychiatrist hugely for. I don’t honestly think an intellectual disability physician would’ve been able to come up with this idea and I’m not sure a consulting psychiatrist would’ve gotten the idea either. Then again, now that I’m on a stable dose of both meds and only wanting to slowly decrease my aripiprazole, my agency’s intellectual disability physician can manage that.

All this combined got me to conclude that I don’t think I want mental health services anymore. If I or any of my current professionals ever change our minds, I can always get referred back to a mental health agency.
